INSTRUCTIONS FOR PLEDGE CARDS
Please print the pledge cards and press hard enough to show up the carbon copies.
Please print out the following information:

Last name, first name, and middle initial.

Employee ID Number.

Department and agency.

Work address including zip code.

Work phone number.

Payroll deduction - the amount of contribution per pay and total pay
(contribution x 26 pay periods = total pay), OR -
Total cash contribution and total gift
(these figures will be the same). -
Five digit agency code
(these codes can be found in the brochure next to each charitable organization listed). -
Annual amount of contribution.

Check a box as to whether or not the contributors would want their name and home address released to the voluntary organization.

Check a box as to whether or not the contributors would want their home email address released to the voluntary organization.

Contributor's signature and date document was signed.
Pink copy goes to the employee.
Yellow copy goes to the payroll office.
White copy goes to the SECC Administrator.
