INSTRUCTIONS FOR PLEDGE LISTING SHEETS
There are 2 types of pledge listing sheets:
The blue sheet is for listing all of the contributors who are making a contribution through payroll deduction.
The green sheet is for listing all of the contributors who are making cash or check contributions.

Be sure to fill out the top section, including who prepared the document, phone number, department/division, address - including city, state and zip code and the date.

Print the name of the contributor along with the total amount of contribution.
